A Record Drug Administration or MAR ( eMAR for electronic version) is a report that serves as a legal record of the drugs given to patients at a facility by a health care professional. MAR is part of the patient's permanent record on their medical chart. Health care professionals come out on notice when the drug or device is given. MAR is usually referred to as drug chart .

The actual charts vary from hospital to hospital and country to country. However they are usually formats:
- Administration/Demographics
- Patient Name (often Last name, First name or similar)
- Treating team details
- Allergies
- Other, variable-weight, special diet, oxygen therapy, time of a local topical anesthetic application, eg. EMLA
- Recipe Details
- Drug name
- Dose strength
- Routes
- Frequency
- Drug Indication/Diagnosis
- Prescribe doctor details, signature
- Graphs day by day in which nurses/nurses who administer drugs can enter when drugs are given

Kardex
Kardis kardex ( plural ) is a generalized trademark for medical administration records. The term is common in Ireland and the UK. In the Philippines, the term is used to refer to the old census chart of the cost nurse normally used during endorsement, where index cards are used, but has gradually been replaced by modern health data systems and pre-printed graphs and forms.
