A Case Statement Information (or Cover Sheet ) is a document submitted to the clerk at the commencement of a civil lawsuit in many US court systems.. These are generally filed along with complaints. Some states use similar documents for criminal cases as well.

Goals and terminology
The purpose of the Case Information Statement is to let the judge and clerk know what type of case the parties are bringing, so they can better prepare the case for trial. Some courts (for example, the New Jersey Court of Appeal) incorporate different types of cases on different "traces", to place limits on how long the invention should take.
If the lawyer fills the Case Information Statement making a mistake, or if circumstances change or new information is found, the party who wants to change the statement can do so by making a motion to the judge handling the case.
Some courts use the term Cover Sheet for this document, but the content and objectives are the same. For example, equivalent documents in federal district court are Form JS-44, Covered Cover Sheet. Similarly, the California High Court has a Form CM-010, a Case Sheet.

Common questions asked in the Civil Civil Information Report include:
- The subject of the lawsuit
- The amount of controversy or solution requested â â¬
- Whether a jury test is requested by either party
- Is there any other party to join
- Whether a lawsuit is a potential class action or other complex case type
- Is there a similar action pending in another court
- What, if any, previous relationships exist between/between the parties (eg work, family, business associates, etc.)
- Whether lawyer fees are in dispute (in some cases, attorneys' fees must be paid by the losers)
In family law cases (such as divorce and parenting issues), the questions raised in the Case Information Statement are often longer and more detailed, requiring reading of each party's work situation, current income, and assets and liabilities, respectively.
Information Information Reports in criminal case
Commonly asked questions in the Crime Information Report include:
- The lawyers involved in the case
- Basic facts and circumstances involved
- Whether the defendant is currently in custody or out on bail
- Is there a defendant's colleague
- Are all issues will be removed by this case
- Whether the constitutionality of an executive law, rule, or order is challenged
New York Request for Judicial Intervention
Documents equivalent to New York are Requests for Intervening Justice, which is necessary because in New York a case begins with the service of complaints and nobody is brought to justice until it is absolutely necessary (usually because discord discovery develops , someone wants to propose a dispositive movement, or actually time for a trial). At that time, the parties seeking judicial intervention of RJI files (who requested information similar to CIS or cover sheet) and files with the court of all relevant documents from the party's own version of the case file.
